Most likely the disk data is corrupted. It's also possible the disk device driver has a bug or the system does not have enough memory.
Some older versions of Windows may show the description as "THIRD_PARTY_FILE_SYSTEM_FAILURE". This is a fatal Windows error, typically called a Stop message, Bug Check, or more commonly the Blue Screen of Death (BSoD). The system is in a forced reboot state. Any unsaved work is likely lost.

A problem has been detected and Windows has been shut down to prevent damage to your computer.
A 3rd Party file system or file system filter has failed in an unrecoverable way. If this is the first time you've seen this Stop error screen, restart your computer. If this screen appears again, follow these steps:
Check to make sure any new hardware or software is properly installed. If this is a new installation, ask your hardware or software manufacturer for any Windows updates you might need.
If problems continue, disable or remove any newly installed hardware or software. Disable BIOS memory options such as caching or shadowing. If you need to use Safe Mode to remove or disable components, restart your computer, press F8 to select Advanced Startup Options, and then select Safe Mode.

Data and/or file structures on the drive may be corrupt. Run the check disk program or other disk validation and repair utility on the drive. To do this:
- Open Explorer or My Computer (Press Windows+E).
- In the right pane, right-click on the drive you want to validate and select Properties.
- Select the Tools tab.
- Under Error-checking, click on "Check Now".

- In the next dialog, make sure the option "Automatically fix file system errors" is checked. If you want a more complete validation and repair check the option "Scan for and attempt recovery of bad sectors" (although this takes considerably longer to complete).

- Click on Start.
Several software products that offer an extensive range of disk diagnostics includes Fix-It Utilities Professional and SystemSuite Professional. Other diagnostic products include Eurosoft's PC Check and Iolo's System Mechanic. |

Update Disk Device Drivers |

The disk (IDE or SCSI) device drivers may have a bug and need to be updated to the current release. |

Install the latest Windows software updates. Don't forget to check for non-critical updates too!
To manually get updates, open Internet Explorer (you can't use a non-Microsoft browser for updates).
For IE 8 and later: Click on Safety, then Windows Update. Follow the instructions.

For IE 7 and older: Click on Tools, then Windows Update. Follow the instructions.

 An unrecoverable problem occurred in a third-party file system or file system filter driver.
After the Stop code will be 4 hex values: {Parameter 1, Parameter 2, Parameter 3, Parameter 4}.
Parameter 1 specifies the the file system that failed. One possible value is 1 to indicate Polyserve (Psfs.sys).
Parameter 2 specifies the address of the exception record.
Parameter 3 specifies the address of the context record.
Parameter 4 is not defined.
